Premium Dental Lab (“Premium Dental Lab”, “we”, “us” or “our”) provides dental laboratory products and services to dental practices and clinicians across Australia. We are committed to protecting the privacy of personal information, including health information, that we handle in the course of our business.

This policy explains how we collect, use, store, disclose and protect personal information, and how you can access or correct your information or make a complaint. It applies to information collected through our website at premiumdentallab.com.au, over the phone, by email, through our order and account forms, and in the course of providing laboratory services to dental practices.

We handle personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

1.  The information we collect

The type of personal information we collect depends on how you interact with us. Broadly, we deal with two categories of individuals:

Dental practices, clinicians and account contacts who order products and services from us, and

Patients whose clinical information is provided to us by a dental practice as part of a lab case, prescription or order.

1.1  Information about dental practices and account contacts

  • Name, job title and practice name
  • Contact details, including phone number, email address and postal address
  • Business and account details, including ABN, billing and payment information
  • Correspondence, order history and case notes relating to your account
  • Website usage information (see section 6, Cookies and website analytics)

1.2  Patient (health) information

Where a dental practice submits a case to us, the accompanying prescription, scan, impression, photograph or clinical note may contain patient identifiers and health information. This may include:

  • Patient name or a patient/case reference number
  • Digital scans, impressions, photographs and shade information
  • Clinical notes, diagnoses, treatment plans and prescribing dentist instructions
  • Any other information the practice includes on a lab prescription form

Health information is “sensitive information” under the Privacy Act and is given a higher level of protection. We only collect this information where it is provided to us by the referring dental practice for the purpose of manufacturing or designing the prescribed dental appliance or restoration, and we do not seek to collect patient health information directly from patients.

2.  How we collect information

We collect personal information directly from you or your practice, for example when you:

  • Create an account, submit a new account form, or request a price list or information pack
  • Submit a lab prescription, order form or digital case file
  • Contact us by phone, email or through our website
  • Browse or interact with our website

We may also collect information from third parties acting on your behalf, such as intraoral scanner or CAD/CAM software providers, couriers, or your practice management software, where this is necessary to process a case.

3.  Why we collect, hold and use personal information

We collect, hold and use personal information for purposes including:

  • Manufacturing, designing and quality-checking dental appliances, restorations and prostheses in accordance with a practice’s prescription
  • Setting up and administering practice accounts, including billing and invoicing
  • Responding to enquiries, providing quotes, price lists and information packs
  • Coordinating courier and freight arrangements for the collection and return of cases
  • Maintaining case records for quality control, warranty and remake purposes
  • Complying with our legal, regulatory and professional obligations, including record-keeping requirements applicable to dental laboratory work
  • Improving our products, services and website
  • With consent, sending marketing communications about our products and services

We do not use patient health information for any purpose other than fulfilling the case for which it was provided, associated quality assurance, and any purpose required or authorised by law.

4.  Disclosure of personal information

We may disclose personal information to:

  • Our staff and contractors who need the information to carry out laboratory work
  • Couriers and freight providers engaged to transport cases and appliances
  • IT service providers, software providers and cloud storage providers who host or support our systems
  • Our professional advisers, including accountants, insurers and legal advisers, where reasonably necessary
  • Regulatory bodies or government agencies where we are required or authorised by law

We do not sell personal information or patient health information to third parties, and we do not disclose patient health information to anyone other than the referring practice, our staff and contractors involved in the case, and our service providers described above, except as required by law.

5.  Overseas disclosure

Some of the service providers we use, such as cloud storage, email or website hosting providers, may store or process information on servers located outside Australia. Where this occurs, we take reasonable steps to ensure

those providers handle personal information in a manner consistent with the Australian Privacy Principles, including through contractual protections where appropriate.

6.  Cookies and website analytics

Our website may use cookies and similar technologies to help it function, remember your preferences, and understand how visitors use the site. We may use analytics and advertising tools, which may collect information such as your IP address, browser type, pages visited and referring website.

You can control or disable cookies through your browser settings, however some parts of our website may not function correctly if you do so.

7.  Direct marketing

We may send you information about our products, services, promotions or industry updates by email or post, where you have provided your contact details to us or otherwise consented to receive such communications. You can opt out of marketing communications at any time by using the unsubscribe link in our emails or by contacting us using the details in section 11.

8.  Data quality and security

We take reasonable steps to ensure the personal information we hold is accurate, complete and up to date, and to protect it from misuse, interference, loss, and unauthorised access, modification or disclosure. These steps include:

  • Restricting access to personal information and patient case files to staff who require it to perform their role
  • Using secure systems and passwords for digital case files and account records
  • Securely storing or destroying physical models, impressions and paperwork that are no longer required, in line with our record-keeping obligations

If we experience a data breach that is likely to result in serious harm to an individual, we will comply with our obligations under the Notifiable Data Breaches scheme, including notifying affected individuals and the Office of the Australian Information Commissioner (OAIC) where required.

9.  Access and correction

You may request access to the personal information we hold about you, or ask us to correct it if it is inaccurate, out of date or incomplete. Requests relating to patient health information should generally be directed to the referring dental practice, as the practice is the primary custodian of the patient’s clinical records. Where we hold information directly and are able to provide access, we will respond to your request within a reasonable period.

We may need to verify your identity before providing access to, or correcting, personal information, and there may be circumstances in which we are permitted by law to refuse a request.

If you have a concern about how we have handled personal information, please contact us using the details below. We will investigate your complaint and aim to respond within a reasonable timeframe. If you are not satisfied with our response, you may lodge a complaint with the Office of the Australian Information Commissioner at oaic.gov.au.
